For patients with a booked appointment next week, regulations require us to have a CoVid screening form filled out a minimum of 2 days before your appointment. You have been sent a link for a web form. Click on the link, fill out the form and hit submit. It will automatically come back to us into your chart. You should receive these forms the same way you get appointment reminders. If you didn’t receive one, please let us know. And please be assured, our staff is screened every day as they arrive at the office to ensure your safety! Thank you for your continued support and patience!
