This wonderful patient of mine e had a #facelift done by another surgeon elsewhere approx 10 years prior. - When she came to see me she expressed some concerns about a scar that is too wide, residual #jowls and lack of definition of her jaw line. Examples of chemical exfoliants include #alphahydroxyacids, #betahydroxyacids, and topical enzymes. - Typical examples of mechanical exfoliants include microbeads, small granules such as #Djoba seeds and ruby crystals and large granules such as abrasive materials like a pumice and magnesium oxide crystals. - So which one is best for you? It depends on your #skintype - Patients with with #acne prone skin will find that mechanical exfoliation can be too rough and might actually spread bacteria which could worsen #breakouts rather than help. Is a male facelift different from a female facelift? - There are some key differences between female and male facelifts, the most important of which is that men typically want a different result from their surgery compared to women. - Female face lift surgery can offer a more delicate, softer appearance while the male face lift surgery when properly performed keeps a rugged or masculine appearance to the face. A female facelift technique applied to a male fe...minizes the face and makes it look awkward. These really are different operations. - Men have different facial anatomy with thicker skin on average. Women tend to have thinner muscular facial layers and connective tissue including superficial fascia compared to men. The skeletal structures also tend to be less prominent compared to men. - Neck liposuction is often included in facelift surgery. With men, the fatty tissue tends to be more dense with more connective tissue making it more difficult to liposuction. For that reason different techniques must be applied to the central neck. - Facial hair is a critical difference. It makes the skin thicker and more vascular which means there’s a slightly higher rate of hematoma’s (blood collections) which must be prevented. We all know #sunscreen is important, but choosing which one to use can be difficult. There are two main types of sunscreens. There are sunscreens that provide a physical barrier and sunscreens that provide a chemical barrier. - Mineral sunscreens work as a physical barrier and chemical sunscreens work to help protect you from the sun‘s harmful rays in different ways. The main type of mineral sunscreen is zinc oxide and titanium dioxide. These work as physical sun ...filters which make them ideal for both daily and prolonged use with intense sun exposure. The pros of mineral sunscreen includefull protection from UVA and UVB rays, it is effective immediately and it will not clog pores. The cons of mineral sunscreen include frequent reapplications and some formulations leave a white cast on the skin - Chemical sunscreens penetrate the top layers of the skin and absorb UV rays. By doing this these filters absorb the damaging UV rays before they can cause damage to the underlying skin cells. The typical chemical sunscreens you will find include oxybenzone,avobenzone, and octinokate. The pros of chemical sunscreen are less product is needed per application to offer protection; thinner and easier to spread on the skin; applies invisibly. Yesterday I made an Instagram TV post where we talked a bit about the Aesthetic triangle. A few people asked me what exactly I was referring to. - As we get older our faces change. In youth, volume is evenly distributed across the facial skin, we have fuller cheeks higher cheekbones, smooth radiant skin and a lean well-defined jaw line. - These features combined create a heart shaped or triangular face that is broad at the top and more narrow inferiorly. Some... people refer to this as ‘the youthful triangle of beauty’. - Facial aging and the loss of these youthful contours (usually caused by volume loss) can cause a the triangle to get inverted. This is often characterized by a sagging facial shape, a wider and slacker jawline, flatter sunken cheeks, forehead wrinkles and the corners of the mouth pointing downwards. - When you think about the aesthetic triangles of the face, you start to realize that correcting the upper part alone will create a disharmonious aesthetic. This is a patient of mine who underwent a #breastlift or #mastopexy procedure. - Her concerns were excessive droopiness of the breast and a loss of fulness in the upper inner part of her breast affecting cleavage - I removed excess skin, corrected the droopiness and resized the nipple areolar complexes.... - Auto-augmentation is when we produce results similar to implants but using only the patient’s own breast tissue. Using an auto augmentation technique, I was able to fill out the superior medial pole of the upper inner breast where she had felt she lost volume. - She liked that the breast is augmented and lifted using her own tissue rather than using any #breastimplants. - Keeping it real: Risks of the procedure include about a 1% risk of minor bleeding, infection and wound breakdown. There are 4 different ways to perform a #breastlift (#mastopexy surgery). - To understand what type of #breastliftsurgery is right for you, you first need to understand #breast drooping (#ptosis). - A youthful breast normally has the nipple and breast tissue sitting above the underlying breast crease, known as the inframammary fold.... - As women age, the breast naturally droops due to the effect of gravity on aging tissues. This is called ptosis. We classify the level of ptosis with a grading system. - 1Grade 1 ptosis has mild breast sagging, and the nipple lies at the level of the breast fold, with breast tissue below it. - 2Grade 2 ptosis has moderate sagging and the nipple tends to lie below the level of the breast fold but still sits in a moderate position on the breast. - 3Grade 3 ptosis is the most severe and involves very significant sagging in which the nipple lies well below the breast fold and the breast tissue is located mostly above the level of the nipple. - With this in mind, someone with extremely mild ptosis and loss of superior volume (for example after breast-feeding) may find a slight breast lift and restoration of volume with a highly cohesive #breastimplant placed in the #subglandular plane. - Someone with slightly more ptosis may require a circumaerolar mastopexy, which involves a scar that goes around the nipple aerolar complex and removes excess skin in that dimension. Because of the circular nature of the well hidden scar, this is sometimes referred to as a #donutmastopexy. - Most commonly someone with moderate breast ptosis would require a scar that goes around the nipple and a straight line down. Some people refer to this as a #lollipoplift due to the nature of the scar. - And lastly those with severe breast ptosis require a scar that goes around the nipple with a straight line down & one that hides within the inframammary fold. Retinoids were first used as an #acne treatment in the early 1970s, but we soon realized that they do so much more. - Patients in the 1970s undergoing retinoid treatment for acne also described improvement of actinic keratosis and evening of #skinpigmentation w...ith improved overall skin quality in appearance. - So how do they work? Retinoids work by increasing the underlying production of #collagen and stimulating neovascularization (the formation of brand new blood vessels) and skin cell turnover. - This minimizes the appearance of wrinkles, bolsters skin thickness and elasticity, and slows the breakdown of #collagen which helps keeps the skin firmer. I’m often asked about fractional lasers. Fractional laser resurfacing works by using heat to create pinpoint areas of injury deep into the skin. We refer to this as microthermal zones. - This triggers the body’s natural healing process to produce collagen and new healthy skin cells. 1000 to 2000 microthermal zones per area are created in each square centimetre of skin treated. - There are two main types of fractional lasers, the more gentle erbium type such a...s the Fraxel laser, and the more aggressive CO2 laser systems. Generally I advise younger patients (those under 50) to use the Fraxel system and similar erbium type lasers for things like moderate acne scarring and find wrinkles. - CO2 laser systems are usually better for older patients with very deep wrinkles who may require a little bit more skin tightening. Why does #dermarolling / #microneedling work on some people and not on others? - Microneedling can be done with a #dermaroller to treat a larger area, or a #dermapen for more precision. I’ll refer to them below as one in the same. - Dermarolling works by creating an injury to the skin with tiny needles. When it is done properly it often produces a red mark and minor pinpoint bleeding. The derma roller microneedle has punctured the skin dermis completely, and th...e result is essentially a #scar so tiny that you can’t see it. - When your body heals from an injury to the dermis it brings in #collagen and growth factors and also stimulates some degree of scar contraction by myofibroblasts. - It is this action that allows #dermarolling to produce improved #skin quality and tightening. This is also one of the ways in which #fraxel lasers work. - If the dermarolling is done very lightly, the results are fairly mild as there is no true injury to the dermis and subsequently no skin contraction or increase in #collagen growth factors. If done at an intermediate level, it can still stimulate some positive changes in the skin. - Derma rolling is best done by an experienced #plasticsurgeon or #dermatologist with appropriate local anaesthesia. It is very important that the right person does your #dermarolling so that when the scar heals it is invisible. Despite looking very natural, the nipple on the second photo is a #tattoo that I did for her while reconstructing the breast. I think it’s important to spread awareness about how far we have come in breast reconstruction! This patient underwent a right sided #mastectomy due to #breastcancer. She had the mastectomy and a tissue expander implant placed elsewhere. She came to... see me with a chief complaint of breast asymmetry, and additional concerns that her implant was high, overly firm and irregular. She was also experiencing discomfort on the right breast due to capsular contracture. I lowered the right impant, reconstructed the nipple and performed a balancing lift on the right side. In the same procedure, I did a tummy tuck with abdominal liposuction. I grafted some of the fat from her abdominal liposuction to the right breast to soften and improve the contour. Fat grafting in this manner also helps to improve discomfort because fat naturally contains #stemcells and when they are brought into a new area they help rejuvenate skin and soft tissue.
