Criteria for return to play following ACL reconstruction may differ depending on your surgeon, but here are a few common goals you should consider: Quad strength >90% compared to the non-injured side. Every 1% asymmetry in quad strength less than 90% = 3% increased risk of further knee injury. Quad:Hamstring strength ratio of <1.5 to 1. The hamstrings are an important muscle for the knee because they act like a secondary ACL. The hamstring is also a common graft for the n...ew ACL, so you want adequate strength in the muscle before RTP. Single leg hop test >90% compared to the non-injured side. As shown in the video, this test involves performing a single leg hop for distance. This testing should occur in a fatigued state. Fatigue can be a contributing factor to ACL injury, so we want to make sure our athletes meet these strength criteria when they're tired, ie worst-case scenario. Sources: Grindem H, et al. Following ACL reconstruction, a patient patient is a therapist's best friend. The decision to return to play (RTP) following an ACL tear and reconstruction is a very complicated one. There are many factors you need to take into consideration before you return to your pre-injury level of activity. A lot of protocols stop after 6 months, however more therapists are opting for a longer timeline, up to 9 months. A couple of studies highlight why being patient with your rehab is b...eneficial: - 38% of athletes who RTP despite not passing RTP criteria reinjure their ACL - Of those who reinjure their ACL, 39% did so when they returned earlier than 9 months - For every 1 month delay in RTP, reinjury was reduced by 51% Even the best ACL rehab still carries a risk of reinjury, but taking your time to ensure adequate strength and control are obtained can greatly reduce your risk of reinjury. Core strength is imperative for anyone dealing with low back pain. After a thorough assessment to determine the cause of your pain and any provoking postures/positions, we begin with some basic core strengthening exercises specific for you. Our core's main job is to protect and stabilize our low back by resisting movement in any of the body's three planes. Each exercise here challenges our core's strength in each plane. 1- plank: resists flexion and extension (sagittal plane)... 2- side planks: resists side bending (frontal plane) 3- pallof press: resists rotation (transverse plane) Each exercise challenges all of the core muscles, just with slightly different bias.
