Lately, I’ve been reading more and more about protein (see my last post), how our guidelines were set for it, and specific amino acids and their roles in health... (especially during pregnancy and early childhood). Suffice to say, we have a lot of re-education around protein and my clinical/personal/anecdotal experience with higher protein being beneficial has a lot of white paper research to support it. I’m not the first person to come to this conclusion and I won’t be the last. The question is how long will it take for our guidelines to catch up? Pictured here: a hearty protein-rich breakfast of chorizo hash (we have a local pig farmer who raises hogs on pasture and makes the most incredible sausage!) with bell peppers and wilted spinach, an egg, and some tangerines. Having enough protein allows me to go all the way from breakfast to lunch with no need for a snack and enough energy to keep up with the kids (or on my work days, enough mental clarity to focus on my work). Hyperinsulinemia is the common denominator of the co-morbidities (obesity, T2DM, CVD, hypertension) in patients who suffer poorer outcomes from COVID-19. Hyper...insulinemia leads to increased inflammation and reduces vitamin D transport, availability and activation. Hyperinsulinemia prevents the breakdown of blood clots. Hyperinsulinemia increases mitochondrial ROS (super oxide, hydrogen peroxide) which leads to increased cytokine production via upregulating the activities of NLRP3 inflammasome & NFkB. Hyperinsulinemia disturbs fibrinolysis via increasing plasminogen activator inhibitor PAI-1, this leads to increasing blood clotting formation - thrombosis, emboli and strokes, not to mention hypoxia. Hyperglycaemia decreases intracellular NAD+ availability, leading to decreased NADPH availability which is essential for the activation of vitamin D. Hyperglycaemia increases synthesis of pro-inflammatory cytokine IL-6 and increases blood coagulation via increasing hepatic clotting factors. Hyperinsulinemia can cause blood glucose numbers to look normal. Hyperinsulinemia can make HbA1c number look normal. Surrogate indicator of normal insulin levels for an individual is ability to produce ketone beta-hydroxybutyrate (BHB) at >0.5 mmol/L throughout majority of the day and 3 hours after a meal with normal glucose levels. Insulin regulates BHB synthesis. Presence of BHB means insulin is below an individual’s pathological high level. To reduce the risk of poorer outcome from COVID-19, target to reduce hyperinsulinemia. The major upstream causal factor of hyperinsulinemia is dietary starchy carbohydrates (bread, pasta, rice, potatoes, fruits like bananas/dried fruits, juices including freshly squeezed and sugar in all its forms) - the dose dictates the poison (if you are restricting carbohydrates below 20 grams/day, and in that you include a teaspoon of honey, this is not going to be a problem). Restricting carbohydrate reduces and over time resolves hyperinsulinemia, this rapidly decreases inflammation and leads to better blood glucose regulation and blood pressure. Three major risk factors - hyperglycaemia, hypertension and blood coagulation/clotting - are resolved with one intervention - carbohydrate restriction. http://dx.doi.org/10.1136/openhrt-2020-001356

False advertising. He loves this shirt even though he doesn’t eat traditional chips and salsa. There are many new followers to this page so I’m going to do a ...very simple breakdown of how we manage River’s diabetes since this gets asked so often in PMs. River uses Novalog in his insulin pump for basal insulin and he uses a combination of R insulin and Novalog for meals. (R is for the protein, Novalog is for the veggies/carbs) R insulin is sold over the counter without a prescription at Walmart for $24.88 a vial. River eats a very low carb diet and the carbs that he does eat are ones that don’t spike his blood sugars. He avoids all grains, starches, sugar, and sweet fruits. His carbs are mostly from vegetables, nuts, and baked goods made using almond or coconut flour. (Like the low carb waffles he eats each day for breakfast.) River eats around 30 grams or less carbs per day and he’s basically gluten free by nature of how we eat even though he doesn’t have celiac disease. We don’t really count carbs. We DO measure protein. For River, every 2 ounces of protein require 1 unit of R insulin. This of course varies by child. We treat low blood sugars with glucose only and we treat the low to bring it back to normal range and not to overshoot that target. 2-3 grams of glucose is all it takes most times. Food is not advised for treating lows as it is slower and less predictable. We treat blood sugars over 100-110 with Novalog. We have found a way to make anything River loves to eat in a low carb version. Pizza, donuts, ice cream, poptarts, hot pockets, and even Oreos can be made low carb. Truth be told, River rarely asks for low carb treats anymore. His palate has definitely changed and he’s satisfied with his savory meals. River plays anywhere between 10-20 hours of tennis a week depending on the season. Because he is so active, he often eats 4 meals a day. We never deprive him of food - so an extra meal gets squeaked in right after school. We avoid having any fast-acting insulin in his body as he’s going into tennis practice. This helps prevent exercise lows. River uses an old Medtronic pump that’s connected to a DIY loop. We only use his loop function overnight to help with exercise related insulin sensitivity and growth hormone spikes. We have a diabetes alert dog who is now a pet because River’s blood sugars are so rarely out of range. Her name is Clover and her rap name is C. Lover. I apologize for the boring details but it will help me to be able to share this post with new families who are asking for an overview of our management strategy. We also follow the principals in Dr. Bernstein’s Diabetes Solution book which I highly recommend. Oh, I can’t forget to add. He’s growing perfectly and his brain is functioning perfectly on a low carb diet. He has all As so far, he’s bilingual, and he takes his classes in Spanish. We’re good here. #letmebe83
