36yo Asian male, about 1900 came back from a run and developed central squeezing chest pain, stated some radiation down to lower arms. Called friend who stated ...patient was initially pouring with sweat on his arrival. On our arrival patient looked well but a little anxious. Not clammy or particularly pale, no difficulty breathing or nausea, does not immediately appear in obvious pain. Denies medical problems or medications. 10 a day smoker and states drinks 2l of whiskey a week. Denies drugs. Average build. Only abnormal ob BP 200/130 ish. Patient had aspirin, 3 or 4 sprays of GTN total and entonox as we were a tech crew. ECGs are on arrival at patient and at cath lab. LAD occlusion, as I'm sure you've guessed! Slightly different angles but the first image with the blue arrow shows where the blockage is, in the second the green arrow points to the reperfused artery with the wire going down it that has now appeared from the point of the blockage.

I have a case that needs some opinion from you all. Feel free to give your ideas. 60 years old male admitted to the hospital after being found high risk anterio...r STEMI. Angiogram showed triple vessel disease and he underwent CABG (LIMA to LAD, SVG to OM, PL) He had PNA complicating along his course but recently he recovered and has been out of ICU already. He has been in the hospital for 3 weeks and is planned for discharge to SNF soon. However, he had the episodes of uncertain tachycardia as attached (ECG 1). The very first one was resolved to NSR after Metoprolol IV was given followed by PO but adenosine 6 mg did not touch anythings. This morning ( ECG 3) he had another similar episode and again responded to Amiodarone + Metoprolol PO What kind of rhythm do you guys think? See more

Interesting one for you all. 84 YOF. Call for 2x episode of Gasping for breath (C1 for ineffective breathing) OA (A)vpu, ABC norm, RR 24, strong reg Radial Pr...esent, other obs nil acute. ECG showing runs of P waves, while these episodes happened radial ceased, heart sounds ceased, PT became quiet but able to respond. PT GCS 14 (dementia) throughout. Rhythm strip showing circa 6 seconds of essential asystole/ventricular standstill. Cardiac centre would not accept as they believed it was a machine/dot/lead error. Eve with multiple FASTecg. 12 lead and 3 lead rhythm strip accompanying. Sorry for the shoddy attempts at blocking all pt identifiable but has to be done.
