This year has been an emotional one that has brought hardship to so many. We feel blessed to have welcomed our son Levi into the world this year, and I am so honoured that many of you have entrusted me with the care of your little ones. Very interesting study on the significance of bio markers that are tested in the Cunningham panel, specifically CaM Kinase II: The significance of this paper is that: a) it confirms prior findings that CaM Kinase II is elevated in PANDAS cases (25 of 35) vs healthy controls (0 of 28)... b) in addition, it highlights that CaM Kinase II is differentiated in PANDAS cases from non-PANDAS OCD, Tourettes and ADHD (especially when evaluating anti-neuronal antibodies in sera) c) it shows that the antineuronal antibodies (particularly anti-D1) and CaM Kinase II elevations are correlated with symptoms in PANDAS cases d) Finally, the paper highlights that when CaM Kinase II activation is elevated in sera, it is likely (94.3%) that CSF will also have Cam Kinase II activation and presence of one or more antineuronal antibodies. This strongly implies that a disruption of the blood brain barrier did occur. Based on the data presented, CaM Kinase II activation is a likely bio-marker for PANDAS.

On Friday we attended a presentation with Dr. Swedo and Dr. Agalliu at BC Children's Hospital -- PANS and PANDAS: Diagnosis, Treatment and Update on Current Research Initiatives Dr. Agalliu presented great research updates showing how repeat Streptococcus pyogenes exposures creates permeability of the blood brain barrier: Frequent Strep exposures in mice lead to the development of Th17 immune cells.... Th17 immune cells and autoantibodies may be important irregularities in the immune system that lead to the pathogenesis of PANDAS and blood brain barrier permeability. Often patients with PANDAS have personal and family history of autoimmune disease, and known history of frequent Strep infection. When the cerebrospinal fluid (CSF) of PANDAS patients were compared to that of their non-PANDAS peers, evidence of "transcytosis" or blood brain barrier permeability could be detected. Goals of the research would be to develop a profile for Th17 cell autoimmunity that could be used as a diagnostic test for PANDAS. For treatment, this research could help in developing new immunotherapies targeted against IL-17A or Th17 cells.
