This tooth was a bit troublesome for me. I couldn’t locate the MB and it wasn’t obvious on the CBCT. It was most likely there mid root. Gentlewave used and i feel it likely addressed any anatomy and that I couldn’t navigate.... hopefully Squirted the canals. A little bit of heroics here as the patient really wanted to keep the tooth despite my emphasizing the survival may be short term. The canals were accessed and covered with Cavit. The tissue was cut away and the tooth was built-up to facilitate rubber dam placement. the canals were reaccessed and Gentlewave was used after shaping with DC 17. The MB and Pa were fused with complex anatomy in the isthmus so this is where Gentlewave may offer an advantage as excess tooth didn’t have to be removed to facilitate this. Single cone with Bioceramic sealer. A fiber post was placed in the Pa and the access closed. This patient had a traumatic injury 13 years ago when he was 18. Mild tenderness and the porcelain has fractured so a new crown is planned. Post removed with ultrasonics and the canal had heavy stained debris. Ran the Gentlewave cycle and the canal looked very clean after as no debris was noted. DT fiber post placed and will check the healing in 4 months. GentleWave case. I know in similar past cases I would have spent 2-3 visits and struggled to get to the apex. Ran the GW cycle after trying to get patency as files weren’t going down. After I was able to get patency and then obturated single cone with BCHFS. One visit. I was a a bit torn on this case. I thought about telling the patient that it would be best to extract this tooth but he wanted to keep it. The survival is short to medium I’m guessing but I proceeded. Not pretty. No pockets greater than 3mm. B.C. sealer used and Biodentine used to seal the pulpal floor. The patient was desperate to keep this tooth as his bridge would be lost. I reluctantly obliged although not sure why i did. The survival will be short but it buys him time.
