Foot FAQ/Fact Friday Q: Why does my child walk with their toes pointed in? A: When your children’s feet point in while walking, this is referred to as in-toeing... or pigeon-toed gait. In-toeing is seen when the thigh or shin bone rotates inwards towards the middle of the body. As a result, the foot points inwards. In some cases, it may be hard to determine the exact cause of the in-toeing as it can work itself out over early childhood development. In other cases, the in-toeing may be coming from a specific factor, such as a compensation of an arch collapse. If the child trips frequently or has developed pain, these are indications that the in-toeing may need to be addressed with corrective measures. These corrective measures may include custom made orthotics, over-the-counter inserts or supportive shoes. These help to support any arch collapse that may be contributing to the in-toeing. In more severe cases, an additional feature called a gait plate may be used on the orthotic to further correct the in-toeing. In addition to pedorthic treatments, manual therapy treatments, such as physiotherapy, may be another helpful way to address in-toeing. Foot FAQ/Fact Friday Q: My heel slides out of most shoes. Any tips? A: Heel slippage tends to be a concern with narrow heels or feet, because the shoe may not... fit as well. To help keep these heels from sliding, there are a few tricks to use! Shoe laces: If the shoe has a second eyelet (lacing hole) at the top, you can feed the lace in that second hole to make it tighter at the heel. The lace lock or heel lock technique is another way to tighten the shoe at the heel. Instead of feeding the lace through the last eyelet on the opposite side, feed it down through the eyelet on the same side, creating a loop. Once you have a loop on both sides, cross each lace over and feed it through the loop on the opposite side. Although this easy to demonstrate in person, it is harder to explain. If you would like more information about the heel lock, talk to your Canadian Certified Pedorthist! Forefoot pads: A forefoot pad is an extra half insert to the front part of the shoe. These prevent the foot from sliding forwards by pushing the foot into the heel, creating a tighter fit at the heel. Heel pads: A heel pad is an extra pad used around the heel of the shoe. This pad is typically not recommended because it pushes the foot forward and can cause issues at the toes. There are some cases where a heel pad can be beneficial, such as a shoe with a very wide heel or very smooth material. Tongue pads: A tongue pad is a pad placed on the inside of the tongue. This acts in a similar way to the forefoot pad, by pushing your foot against the heel of the shoe. Foot FAQ/Fact Friday Q: As a diabetic, how can I protect my feet at home? A: With diabetes, it is important to protect your feet at home to avoid preventable ...foot issues. An easy way to do this, is to wear indoor footwear. Wearing footwear in the house protects the feet from injury, such as stubbing your toe or stepping on a loose object. It also protects the feet from pressure related injuries from the way you walk. Indoor footwear can be a slipper, sandal, running or walking shoe. Each style has a different level of support. To choose which style is best for you, talk to your Canadian Certified Pedorthist as there are multiple factors to take into consideration. In all these indoor footwear options, there are some additional important features to consider as a diabetic. Make sure there is support at the heel. This can be a back strap on a sandal, or a backing to a slipper or shoe. For additional support, make sure the heel counter is solid by pushing on the back of the heel and feeling resistance, instead of just soft material. Look for a removable insert. In many cases, a supportive insert is necessary to prevent excessive pressure points on the bottom of the foot. The removeable inserts allows a custom orthotic or an over the counter insert to be used indoors. Be aware of seams and hard spots on the interior of the shoe that could irritate the skin. Feel the inside of the shoe to check for these areas. Also, be aware of any straps that may irritate a problem spot or a bony prominence. Ensure a proper fit. If the shoe is too tight or loose, pressure or rubbing can cause issues to the feet. To find the proper indoor shoe for you or someone close to you, talk to a Canadian Certified Pedorthist! Foot FAQ/Fact Friday Q: Can you wear orthotics in skates and ski boots? A: The short answer is yes. ... No matter the activity, if your feet need some extra support or cushioning a custom orthotic can be made to work with your feet and fit in the appropriate footwear. Skates and ski boots require a lower profile orthotic that has a shape that reflects the contours of the inside of the footwear. Because skates and ski boots generally have a firm/stiff boot they act differently then your regular day to day footwear. Having orthotics made specifically for this type of footwear means you can put them in and leave them there. In addition to the unique fit requirements, the actions that your feet perform inside skates or ski boots can be very different from those in your street shoes and the orthotics may require different specifications. Your Canadian Certified Pedorthist will ask you to bring in your skates or ski boots to your initial assessment and when fitting the orthotics. Some C. Ped (C)s are also skilled in modifying your winter sports footwear for things like bony prominences. In most cases you’ll need to have your custom orthotics made specifically for your skates and ski boots as it is unlikely that they will be interchangeable to other footwear like running shoes or sandals. Due the colder weather, surfaces harden and create more lumps/dips. These shoes will provide a stable sole with integrity. Damp conditions can lead to unwanted pain, swelling and even blistering. Try to select a water resistant, or Gortex lined, water proof sole. If your occupation requires specific footwear as a uniform, consider commuting in a shoe most ideal for the weather conditions. Fall is a good time to put the sandals away due to the cold weather. The temperatures may warm up during the day, but forcing your muscles and tendons to perform immediately without the proper blood flow can lead to vascular and soft tissue injury to your lower limbs. Look for footwear with temperature regulating features and footwear that fits properly. There should be enough wiggle room for the toes as a tight fit can create numbness, which can make your cold feet even worse. Try to avoid rain boots, military style boots, canvas and low-profile shoes, especially during longer periods of time. Typically, these shoes are not orthotic friendly and do not contain features to help with pain. Look for shoes with a stable shank, firm heel counter, adjustability and a stable sole.
