Much in the news about LTC facilities centres on profit vs not-for-profit. This isn't the dialogue we need to have. We need to rethink the very premise of LTC facilities. The folks who live in them are not sick. They have chronic and disabling conditions and require support with everyday activities. They deserve a home like environment in which to have this support, not facilities based on an outdated medical model of "care". This represents a fundamental structural change.... Facilities based upon economies of scale in which larger is better only incidentally support individuality of support and well-being--it's not the primary purpose. Pat Armstrong (York University) and others have examined the impact of environment on well being for both the folks who live in a setting and those who support them. Many countries in Europe have embraced approaches that use smaller homes with good result. As Ontario prepares for some sort of review of LTC facilities, we need to make this clear. The answer isn't to build more 300-person facilities. It lies in building homes of much smaller size that support the persons who live there, not simply manage a list of diagnoses.
